Cancellation and No Show Policy

Cancellations and no shows are very hard on a mobile physical therapy practice. Please keep the following cancellation policies in mind:

• Please give me notice a week in advance of any schedule changes if possible.

• Cancellations with less than 24 hours notice have a $40 fee

• No shows have a $40 fee

• Patients with multiple cancellations will be given one warning and then removed from the schedule if cancellations continue to be frequent
